Role: ST4 paediatrics. Setting: Baby clinic. You are talking to Joel and his mother Sophie. Task: Joel is 18 months old. He is the second child to his parents. He was born by normal vaginal delivery following a twin pregnancy at 37 weeks. Please assess his fine motor skills. Attempt this task before looking at the answer/video

From the age of six months, children progressively develop more precise manipulative skill.

Introduce yourself to the child and parent and gain rapport. Sit the child on an appropriate chair or in their parent's arms. Offer to test their vision and ask the parent if they have any concerns with their child's vision.

Offer the child a piece of paper that has been wrapped up into a small ball. Comment on their pincer grip.

Offer the child a brick and comment on the following- their ability to release intentionally, can he/she bring two bricks together? What does he/she do if you offer her/him a third brick or a second brick?

Build a tower with some bricks and see if the child can copy it.

Draw a circle or a few lines on a piece of paper and ask the child to copy it. Comment on the way in which they hold the pencil and what they draw.

Offer the child a book. Observe and comment on how he/she opens the pages. One  page at a time or many pages at a time.

Ensure you offer one task at a time to the child and your table is not crowded as this will distract you the child and maybe the examiner!

Summarise your findings and present them to the examiner. Make a conclusion on the child’s development. Say whether it is appropriate, delayed or advanced for the child’s chronological age.
