Dear Paul,

if there are no other structural anomalies on the scan and serum sreening is low risk, am I right in understanding that karyotyping is not necessary even if maternal age is advanced?Of course maternal request is important but we do not need karyotyping purely on one of the soft markers and advanced age?

Serum screening takes account of maternal age. So if serum screening is available, you do not need to consider maternal age as an independent factor. However, if serum screening is not available, then it would be appropriate to consider age-related risk.
