Instructions

For each scenario described below, choose the single most appropriate clause under which pregnancy may legally be terminated in the UK from the above list of options. Each option may be used once, more than once, or not at all.

1) A healthy 18 year old woman requests termination of pregnancy at 8 weeks gestation for personal reasons


2) A 42 year old woman with pulmonary hypertension is referred to the clinic at 6 weeks gestation.

3) A 20 year old woman is found to have a risk of having a baby with Down’s syndrome screening of 1 in 2 at 18 weeks gestation. The fetus is found to have an atrio-ventricular septal defect and duodenal atresia on ultrasound scanning.

4) A 32 year old mother of 4 children attends for anomaly scanning at 20 weeks gestation. No anomalies are detected. She becomes very upset when she is informed that the fetus is female and requests termination of pregnancy 1 week later

5) A 32 year old woman was found to have a fetus with Down’s syndrome on amniocentesis at 19 weeks gestation but decided to continue with the pregnancy. At 30 weeks gestation, she requests termination of pregnancy.

The Abortion Act 1967

The Abortion Act 1967 forms the basis on which a pregnancy may legally be terminated in the UK. There are 7 Clauses or Grounds on which a pregnancy may legally be terminated.

A: The continuation of the pregnancy would involve risk to the life of the pregnant woman greater than if the pregnancy were terminated

B: That the termination is necessary to prevent grave permanebt injury to the physical or mental health of the pregnant woman

Note that no gestation age limit is stipulated in Clauses A & B.

C: The pregnancy has NOT exceeded its 24th week and that the continuation of the pregnancy would involve risk, greater than if the pregnancy were terminated, of injury to the physical or mental health of the pregnant woman (includes pregnancies up to 24 weeks + 0 days) This clause forms the basis for so-called ‘social’ abortions

D: The pregnancy has NOT exceeded its 24th week and that the continuation of the pregnancy would involve risk, greater than if the pregnancy were terminated, of injury to the physical or mental health of any existing child(ren) of the family or of the pregnant woman (includes pregnancies up to 24 weeks + 0 days)

E: There is a substantial risk that if the child were born, it would suffer from such physical or mental abnormalities as to be seriously handicapped This clause forms the basis for terminations for fetal abnormalities. NOTE THAT NO GESTATION AGE LIMIT IS STIPULATED IN THIS CLAUSE

F and G are for emergency terminations only. F: To save the life of the pregnant woman; G: to prevent grave permanent injury to the physical or mental health of the pregnant woman

Question 6 – D: TOP at 10 weeks should be surgical by suction evacuation. Mifepristone is only licensed for use until 9 weeks (63 days from LMP).

Question 7: B: Less than 9 weeks so medical TOP would be appropriate (and you would want to avoid GA). Misoprostol does not cause bronchospasm and can be used in asthmatics

Question 8: K: Fetocide recommended though not essential. Mifepristone (oral) + vaginal misoprostol should be used

Question 9: D: Mifepristone contra-indicated in renal impairment. Therefore surgical TOP would be most appropriate method as prostaglandins on their own associated with prolonged initiation – to – termination interval and high failure rate.

Regarging the answer to question 4 , the mental health of the lady will be affected if she will continue pregnancy (cluase C ) . She is still less than 24 ws . Would it be possible to allow her to terminate the pregnancy if she didn t tell us about concern abuot the fetal sex ?

Regarging the answer to question 4 , the mental health of the lady will be affected if she will continue pregnancy (cluase C ) . She is still less than 24 ws . Would it be possible to allow her to terminate the pregnancy if she didn t tell us about concern abuot the fetal sex ?

The question makes it clear she is terminating the pregnancy because of fetal sex and this is not allowed no matter how distressed she will be about having a baby with the \'wrong\' sex
