Hi Dr Ayuk , I've come across a question where a lady aged 32 has sickle cell disease and is 8 weeks pregnant. Her bmi is normal, no other risk factors. The question is about thromboprophylaxis. I thought that SCD gets a score of 3 therefore antenatal lmwh from 28 weeks and 6 weeks postnatally but the answer is antenatal prophylactic lmwh and 6 weeks postnatally . Could you please explain why it wouldn't be from 28 weeks only

Thanks - you need to post the entire question with the options. You have posted your interpretation of the question which is not necessarily accurate

A 32 year old woman attends the antenatal clinic at 8 weeks gestation in her first pregnancy. She has a history of sickle cell disease . Her bmi is 24.

Thanks - I am afraid the answer is correct as stated in the explanation based on RCOG 2015 guidelines

NB If persisting or > 3 risk factors consider extending thromboprophylaxis with LMWH

Please see the flowsheet on page 33 of this document

6 weeks is for women with high risk factors. For intermediate risk factors >3 it clearly states CONSIDER EXTENDING DURATION.

 

Option B says at least 10 days which is correct.
