There is no single 'correct' answer to a question like this and you are asked to SELECT SQL_CALC_FOUND_ROWS the 'best' answer from the list of options. We have similar questions in the Clinical Governance module with an explanation of the principles guiding your answer. As the educational supervisor you will have to take action which would be visible on the trainee's portfolio so any option that does not include documentation in the portfolio would be wrong. Any options involving external organisations (GMC, BMA, Police...) will also be wrong. The question for the educational supervisor would be when to involve the College Tutor / Deanery.
