1) A general practitioner telephones for advise about a 51 year old woman who had a the MIRENA IUS inserted at the age of 47 years for contraception and heavy menstrual bleeding. The woman has not had any menstrual periods for 3 years and is considering removal of the intra-uterine device.

A) Replace IUS now

IUCD (MRCOG II July 2018)

•    Mirena is licensed for 5 years for contraception and for HMB and for 4 years for endometrial protection during oestrogen-only HRT.

•    The Mirena LNG-IUS may be used for contraception until the age of 55 if inserted at age 45 or over even if the woman is not amenorrhoeic

•    Women using Mirena for HMB only can keep the device in situ for as long as it is effective, irrespective of the age at which it was inserted

•    Women using Mirena for endometrial protection in an HRT regimen must have their device changed every 5 years.

shouldnt choice of 1. be H. and Choice of 2. A  as oestrogen only HRT is being given , hence mirena now must be changed as it will not give endometrial protection
